      Objective To investigate the medication adherence of patients with severe mental disorders and its influencing factors in Zhengzhou. Methods This study was a cross-sectional research. From March 2021 to June 2021, a self-designed questionnaire was administered to 342 individuals selected from all registered patients in Zhengzhou in the National Information System for Severe Mental Disorders using a stratified multistage cluster sampling method. The questionnaire included general demographic items and a self-designed questionnaire on factors influencing medication. Results A total of 320 patients were included in the final analysis. The medication adherence proportion of patients with severe mental disorders was 76.56%. The differences between patients in the medication adherence group and those in the medication non-adherence group were statistically significant in terms of residence, occupation, outpatient chronic disease reimbursement, insight deficit, stigma, and medication-related factor (2=14.015, P<0.001;2=7.502, P=0.023;2=13.106, P<0.001; Z=7.588, P< 0.001; Z=2.379, P=0.017; Z=2.893, P=0.004). Outpatient chronic disease reimbursement was a promoting factor for medication adherence (OR=2.727, 95%CI1.320-5.634), and rural residence and insight deficit were risk factors for medication adherence (OR=0.465, 95%CI0.221-0.977; OR=0.398, 95%CI0.286-0.553). Conclusion Patients with severe mental disorders in Zhengzhou have a high proportion of medication adherence, and increasing the proportion of patients with outpatient chronic disease and improving the level of insight will further improve medication adherence.
